The origin is unusually traceable. Japan's big message boards had a mental health section, メンタルヘルス板, and its regulars were nicknamed メンヘラー, then shortened to メンヘラ in the late 2000s. The word left the board and kept going, which is how a term for a forum population became a term for a personality.
That history is worth knowing, because it explains why the word can cut. Aimed at somebody else it can be genuinely cruel, and a whole aesthetic, all pastel and self-aware sadness, has grown up around it too.
