The verb belongs to sickbeds. こじらせる means letting something get tangled and worse — a cold you never rested off, a negotiation that soured — and behind it sits こじる, to twist or pry at. Somewhere along the way people started aiming it at themselves, and the metaphor was too good to give back. Feelings, left untreated, do knot.
